Return Parts Authorization Form

To return Spare Parts, please fill out and submit the Return Parts Authorization Form below.

After you submit the form, you will receive an email from the website confirming your request.

Once the RMA is processed and authorized you will receive another email with the RMA file with the RMA#.

Please print the file and include it in the shipping package.

Please ensure you fill in all information below for timely crediting. Your account number, and either an order number or invoice number will need to be provided. If the return goes to multiple orders or jobs, please make sure you fill a separate form for each order.

  • All new and unused parts returned are subject to a 20% restock fee.
  • No parts will be accepted as returned without this form COMPLETED entirely.
  • If “Reason for Credit” is not filled out a 20% restocking fee will be applied.
  • No electronic parts will be credited for any reason
  • Any electronic parts being returned must be approved by customer service.
  • Defective parts accepted for return will be subject to replacement only. No credit will be given.
  • No consumables will be accepted as returns.
